Bridging the Dichotomy: A Medical Researcher’s Journey from Western Pharmaceutics to Acupuncture
In Malaysia, there was once a prevailing mindset, particularly among the English-educated, that conventional Western medicine was the only legitimate recourse for illness, often dismissing traditional Chinese medicine (TCM) practitioners, historically known as "sinsehs." As a clinician and senior medical researcher, my recent personal health experience completely upended this conventional dichotomy. Suffering from excruciating neck, shoulder, and arm pain—clinically presenting as cervical radiculopathy from a compressed nerve root, I deliberately chose acupuncture over standard pharmaceutical interventions. From a pharmacological standpoint, I wanted to avoid the routine prescription of NSAIDs like Ibuprofen, Celecoxib, or Indomethacin, which inevitably require pairing with a proton pump inhibitor (PPI) simply to mitigate the high risk of gastric ulceration and mucosal bleeding.
My first choice of a TCM centre was fully booked for two weeks, a striking testament to the surging demand for integrated therapies. This high clinical volume for appointments for TCM or other complementary systems of medicine aligns with World Health Organization (WHO) epidemiological data showing that 80% of the global population relies on traditional or complementary medicine, a demographic that, to my knowledge, includes many of my former clinician colleagues, including a highly qualified former Senior Consultant Psychiatrist who was formerly Head of the Dept. of Psychological Medicine at the University Hospital, University of Malaya, who later became a Consultant to WHO - whom I knew since our university days, and now, myself.
I eventually consulted a husband-and-wife team of TCM practitioners whose academic credentials rivaled Western medical training: five-year Bachelor’s degrees in TCM from UTAR, supplemented by two years of rigorous clinical residency in Chinese hospitals.
The practitioner treating me executed a clinical protocol that mirrored conventional Western diagnostics. He began with a meticulous medical intake, documenting my comprehensive history and recording detailed clinical notes. After checking my blood pressure, he performed a traditional tongue diagnosis, analyzing specific topographical regions mapped to internal organ systems, where the tip reflects the heart, the sides reflect the liver, and the back reflects the kidneys to evaluate my systemic Qi and fluid balance.
Moving to the examination room, he palpated the musculature of my neck and arms, conducting a thorough range-of-motion assessment. He tracked my symptoms using the standard 1-to-10 visual analogue scale (VAS)—the exact pain scoring system we utilize in conventional hospital settings. As a researcher, I was profoundly impressed by this structured, scientific methodology.
My 20-minute acupuncture session was paired with an infrared lamp, a combination with clear physiological rationale. The thermal energy boosts localized microcirculation and reduces hypertonicity in tight muscles. By penetrating deep into the skin, fascia, and underlying tissue, the infrared heat expands micro-vessels, accelerating the delivery of oxygen-rich blood to the injured site. Furthermore, this gentle heat activates peripheral thermal receptors to downregulate and alter pain signals traveling to the spinal cord—perfectly validating the gate control theory of pain. Having noted a significant therapeutic response and symptom reduction after just the initial treatment, I am fully committed to completing my remaining four or five follow-up sessions.
